When you schedule your chiropractic appointment over the phone, make sure to mention the car accident consultation and exam. When you schedule using the online form, select “Comprehensive Auto Accident Exam” under Appointment Reason. This will help us make sure your appointment is scheduled correctly. 

After scheduling your appointment, make sure you fill out the Car Accident History Form for New Patients and bring it with you to your first visit.
